About the role

  • Liaise with inpatient clinical teams and hospital palliative care services to optimise care transitions and home-based care.
  • Provide undergraduate and postgraduate teaching, research, quality assurance audits and continuing education as integral to the position.
  • To liaise with palliative care multidisciplinary colleagues across MNH, to streamline the patient's journey and ensuring care is delivered within their chosen location whenever possible

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Provide clinical Palliative Medicine/Palliative Care to hospital inpatients and outpatients.
  • Optimise clinical handover and documentation.
  • Attend and contribute to multidisciplinary team meetings and case conferences, providing expert advice related to the management of and support to patients requiring palliative care.
  • Liaise with all disciplines of staff involved in the patient's care and clinical management to optimise timely therapeutic interventions, discharge planning and end of life support.
  • Provide clinical supervision to allocated advanced trainees, basic trainees or prevocational junior doctors.
