About the Role
Bolton Clarke enrolled nurses provide a range of clinical and care services to our Home Care clients across a variety of practice areas, assisting them to experience a greater level of independence and ensuring their health and wellness are supported.
Responsibilities include:
- Providing nursing care with the support from an experienced Registered Nurse
- Collaborating with community supports, referrers, GPs, families and allied health
- Contributing to the assessment, prevention and early detection of any health conditions
- Contributing to the formulation of care plans, reviews and case conferencing
Work Schedule & Coverage
Primary Hours: Monday to Friday, 7:30am – 3:00pm / 4:00pm
**Monthly Rotation:**One evening shift & one weekend shift per month
Location: Service clients across the Eastern suburbs including
Balaclava, Elwood, Ripponlea, St Kilda, Prahran, South Yarra, Windsor, Albert Park, Middle Park, Port Melbourne, South Melbourne and surrounding suburbs
About You
To be successful in this role, our preferred candidate will have:
- Relevant qualifications and AHPRA registration
- Minimum 12 months experience as an Enrolled Nurse (community or acute setting preferred)
- Excellent communication, adaptive thinking, planning and prioritisation, and cross-functional team working skills
- Valid working rights in Australia
- Police Check (valid within the last 3 years)
- Driver's licence and access to a reliable vehicle for work
