About the Role

We are seeking experienced and compassionate Registered Nurses to join our Home and Community Care team. In this role you will deliver a broad range of clinical care services in the community, supporting clients with their health and wellbeing in the comfort of their own homes.

Your responsibilities will include:

  • Working independently with support from experienced senior clinicians.
  • Implementation and monitoring care of clients and their carer/s
  • Developing appropriate therapeutic and caring relationships
  • Collaborating with likeminded health professionals to achieve client outcomes
  • Participating in the development of a supportive environment with colleagues

Work Schedule & Coverage:

  • Primary Hours: Monday to Friday, 7:30am – 3:00pm / 4:00pm
  • **Monthly Rotation:**one evening shift and one weekend per month
  • Location: Service clients across the suburbs of Balaclava, Elwood, Ripponlea, St Kilda, Prahran, South Yarra, Windsor, Albert Park, Middle Park, Port Melbourne, South Melbourne and surrounding suburbs

About You

To be successful in this role, our preferred candidate will have:

  • Relevant qualifications and AHPRA registration
  • 12+ months experience as a Registered Nurse (community or acute setting preferred)
  • Excellent interpersonal and communication skills- written and verbal
  • Excellent organisation skills, with the ability to plan and prioritise
  • Proven ability to contribute to and, work as part of a dynamic team under direct and indirect supervision and within scope of practice
  • Police Check (or willingness to obtain)
  • Driver's licence and access to a reliable vehicle for work
