  • Full-time, contract until June 2027 | Based on Bidjigal Country in Hurstville
  • $76k - $95k + Super + Salary Packaging (depending on skills and experience)

About the role

As our Young Carer Case Manager, you will work alongside young carers to deliver meaningful, person-centred support that helps them navigate their caring responsibilities while continuing to grow, connect and thrive.

As part of the Carer Gateway program, you will support young people under 25 through a dynamic mix of case management, peer support and community engagement - building connection, strengthening resilience and improving overall wellbeing.

This is a varied and purpose-driven role, well suited to someone who is motivated, emotionally intelligent and keen to develop their practice while making a tangible difference in the lives of young carers.

What you get to do

  • Provide end-to-end case management support to young carers, including intake, assessment, goal setting and ongoing service reviews.
  • Empower young carers to navigate their caring responsibilities while developing the confidence, self-esteem and social skills to engage in school, social life and relationships beyond their caring role.
  • Collaborate with other team members to develop and deliver tailored programs and activities that meet the specific needs of Young Carers.
  • Engage with the community to raise awareness of supports available to young carers and increase referrals, including attending interagency meetings, delivering presentations, and hosting stalls at school and community events
  • Manage a varied caseload and competing priorities across case work, group facilitation and outreach, ensuring timely and high-quality service delivery.

We want you to be successful in your new role! This means you’ll have:

  • Demonstrated experience as a Case Worker, Case Manager or Social Worker working with young people.
  • Tertiary qualifications in social work, psychology, community services or a related field.
  • Strong emotional intelligence, with the ability to respond to individuals experiencing stress, uncertainty or vulnerability.
  • Demonstrated ability to build rapport and communicate effectively with young people from diverse backgrounds.
  • Ability to manage competing priorities and work across multiple aspects of service delivery, including case management, group facilitation and community engagement.
  • Demonstrated ability to develop person-centred service plans in collaboration with young carers, families and relevant stakeholders, ensuring alignment with individual needs, goals and eligibility.
  • Comfortable working from our Hurstville office and travelling to inner, northern, eastern and western suburbs areas of Sydney and Wollongong for community outreach.
