About the role
As our Care Partner (Registered Nurse) you’ll support older people across Greater Sydney to live with independence, dignity, and quality of life. This role has a strong clinical focus, working with clients who have complex and evolving needs, including those requiring restorative care and end-of-life care.
You will use your clinical expertise to assess needs, develop tailored care plans, and guide care delivery that aligns with each client’s goals, preferences, and stage of life. Working closely with Community Care Workers and a broader multidisciplinary network, you’ll ensure services are coordinated, responsive, and aligned with the consumer-directed care model.
Key Responsibilities
- Provide phone-based case management and support to clients under the Commonwealth Home Support Programme (CHSP) and Support at Home (SAH), including completing assessments and developing personalised care plans.
- Apply a restorative care approach to support clients in maintaining or improving their independence and function.
- Use a relationship-focused approach to build rapport over the phone and identify each person’s goals, needs, and strengths.
- Build and maintain strong networks with agencies, health professionals, and community services, including ACATs, GPs, social workers, and culturally specific organisations.
- Deliver person-centred case management in collaboration with clients, families, and carers, primarily through phone-based engagement.

Skills and Experience
- Registered Nurse (AHPRA) with experience in case management within ageing, community care, or a related setting.
- Demonstrated experience working within Support at Home (SAH) and/or the Commonwealth Home Support Programme (CHSP).
- Strong ability to deliver person-centred, holistic care, supporting clients with complex needs, including restorative and end-of-life care.
- Excellent interpersonal and communication skills, with the ability to build trust and engage effectively with people from diverse backgrounds.
- Proven ability to work collaboratively within a multidisciplinary environment while managing competing priorities.
- Highly organised, with strong time management skills and the confidence to represent the service at community events.
- Current driver’s licence (essential).
