Your opportunity
Our Community Living Support (CLS) service is a recovery-focused program designed to support individuals experiencing mental health challenges to live well in their communities. The service provides tailored, strengths-based support that promotes independence, builds daily living skills, and connects people with the services and networks they need to thrive.
We are looking for a Senior Recovery Case Worker to play a pivotal role in supporting people to rebuild their lives and achieve lasting wellbeing. In this role, you will lead the delivery of person-centred, recovery-oriented support to individuals with complex needs; helping them overcome barriers, strengthen independence, and reconnect with their community. Working collaboratively with a range of internal and external stakeholders, you will bring both expertise and compassion to complex case coordination while contributing to a high-performing, values-driven team.
Your key responsibilities will be to:
- Deliver high-quality, person-centred case management to individuals with complex needs using a recovery-oriented approach
- Conduct client intake, assessments, and develop holistic, strengths-based case and safety plans
- Provide outreach and practical support to assist clients with daily living and independence
- Coordinate care, referrals, and stakeholder collaboration to achieve positive client outcomes
- Facilitate group programs and contribute to the development of client support activities
- Maintain accurate documentation, case notes, and reporting in line with compliance requirements
- Build and maintain strong relationships with clients, colleagues, and external service providers while providing guidance and mentorship within the team
Requirements for success
- Relevant qualification in community services, mental health, or a related field (or equivalent experience)
- Current and valid Working with Children Check and a valid Driver’s licence
- Demonstrated experience supporting people with complex needs in a community or mental health setting
- Strong understanding of recovery-oriented practice and person-centred approaches
- Proven ability to manage complex caseloads and make sound, independent decisions
- Excellent communication and relationship-building skills with clients and stakeholders
- Ability to work autonomously while contributing positively to a team environment
- Strong administrative and digital skills, including case management systems and reporting
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
