As a Care Finder Support Worker (Case Manager), you will deliver intensive, end-to-end support for older people with complex needs—navigating aged care, health and community systems where standard pathways don’t work. You’ll work in the community, building trusted relationships, and ensuring people are supported to access aged care services and other relevant community support.
Your Opportunity
This is an 18‐month, part-time position with a total commitment of 19 hours per week, to be worked across Tuesday, Wednesday, and Thursday. The role is based in the Lower Blue Mountains region (Penrith office).
In this role, you will support people within the Care Finder target population by engaging through assertive outreach, building trust, and guiding them through My Aged Care and related service pathways. You will work collaboratively with health professionals, community organizations, and government agencies to ensure people receive the right support at the right time.
This is a highly autonomous, community-based role suited to experienced professionals confident managing complex cases and multi-agency coordination.
With empathy, professionalism and strong advocacy skills, you will:
Lead assertive outreach for hard-to-engage clients
Establish trust with highly vulnerable clients with complex barriers (mental health, social isolation, system disengagement)
Coordinate across health, aged care and community systems to resolve complex access challenges
Respond to enquiries and intake applications promptly, providing warm handovers where required
Assist clients to reconnect with My Aged Care or alternate providers when circumstances change
Ensure clients receive the right supports to achieve their goals and improve wellbeing
Maintain regular check-ins once services commence
Manage a full caseload within organizational guidelines
Contribute to continuous improvement across program policies and practices
Participate in WHS activities and promote a safe work environment
What You Will Bring
You are compassionate, proactive and committed to supporting older Australians who may be vulnerable or facing complex barriers to care. You enjoy community-based work and advocacy.
You will also need:
- Tertiary qualifications in Community Services, Welfare, Aged Care, Social Work or a related field
- Exposure to government-funded programs (Care Finder / My Aged Care)
- Experienced professional in Home Care and Aged Care
- Case management or care coordination experience
- Strong communication, negotiation and problem-solving skills
- Confidence engaging with a wide range of stakeholders and service systems
- Intermediate computer skills (MS Office and CRM systems)
- Current NSW driver’s license and own car
- To be able to present to key stakeholders for the region alongside team members or doing this independently
This role suits someone comfortable working independently in complex, sometimes ambiguous situations, advocating for clients who may not engage with traditional services.
